Medication is first-line for IPSS 8–19. Procedural intervention (UroLift, Rezum, TURP) is considered for: IPSS 20+, medication failure or intolerable side effects, or BPH complications (acute urinary retention, recurrent UTI, bladder stones). Most men with IPSS under 19 can be managed with medication.
